Everything described above sounds clean in retrospect. The build was messy. Here’s what actually breaks, and what fixes work.
When CLAUDE.md reaches 300 lines and you have 21 domain rule files, instructions conflict. The fix is progressive disclosure: load overview always, domain rules only when triggered.
Nodes reference deleted files. Relationships point to renamed documents. Without weekly audit, graph quality degrades within 2-3 weeks.
5 agents sharing 1 repo can step on each other. Branch state becomes unpredictable. 3 failed architecture approaches before finding patterns that work.
Giving the AI everything makes it worse. At a certain volume, signal-to-noise drops below useful. Agent task sizing is a real constraint.
